JohnnieI found these to be extremely tight in the heel initially. I needed a shoe horn to get my feet in at first. Break-in was very easy and after the shoes stretched a little, I found them to be extremely comfortable for all day wear. The insole is like a sneaker insole, it's padded, covered in knit synthetic stuff and removable. I assume the shoe would accommodate an orthotic with the insole removed. Nice modern loafer silhouette from Rockport with a subtle style. The thin outsole is not too chunky in the width; squared-off toe is slightly long if you buy your size. I got the smooth leather version. The supple leather is obviously of good quality. These are nice casual shoes that could be worn dressier in a pinch if you have the style to pull it off.
